"What If" takes an abrupt turn into powerful emotional "reality" once Dean figures out what's going on and his newfound happiness is shattered by the grim realization that the many losses his family has suffered in the "real" world are important and necessary. "Why do we have to sacrifice everything?" Dean sobs at the grave of a loved one. He's crying for his family's sacrifices, for lost happiness and because for the first time he truly understands the overall importance of what the Winchester men do. (The emotions Dean is feeling in this scene surface once again at the end of the episode.)
